MVSL Referee Evaluation Form - Fill out this form after each game and email it to Ana Ostos at registrar.mvsl@outlook.com.  We use these evaluations to weed out the bad refs and note the good ones.  It’s a very valuable tool to make sure you have the best referees reffing your games.

A CONFLICT FORM no longer needs to be filled out. Please email the Scheduler, David, with your conflicting dates/times and the names/dates of the tournaments your team will be attending during the season. Our Scheduler will do their best to accommodate.  el.sanch.ds@gmail.com
